What to bring
Bringing a few things on your first visit helps us see you faster and treat you better:
- Photo ID and insurance card
- Any X-ray films, MRI discs, CT scans, or other prior imaging
- Copies of relevant medical reports (X-ray, MRI, CT, EMG, bone scan)
- Any referral your insurance requires
- Your patient co-pay as outlined by your plan
- Your referring physician's name and phone number
- A list of current medications and known allergies
New patients: please arrive 30 minutes early to complete intake paperwork in the office. Returning patients can arrive 10 minutes early.
When we're open
Monday through Friday: 8:00 AM to 5:00 PM at all eight locations.
Saturday and Sunday: Closed at all locations. For weekend emergencies, please visit your nearest urgent care or emergency room. We'll follow up on Monday for any continuing care.
Holidays: closed on major holidays. Same-day urgent slots are still available the business day before and after.
